The PCMHI Psychologist will function within a fast-paced primary care team and can expect to consult with eight to ten patients per pay with the primary goal of assisting Patient Aligned Clinical Team (PACT) members with identification, treatment and management of mental health and behavioral medicine conditions in the enrolled population.
The PCMHI Social Worker provides education and brief interventions to patients, monitors response to and side effects from treatment, and provides education and support to patients. The PCMHI Social Worker also provides "on-time, on-target" information to Primary Care Providers (PCPs) and Behavioral Health Services (BHS) staff, and collaboratively makes appropriate care decisions.
