The Primary Care Mental Health Integration (PCMHI) Psychologist for the Wilmington Veteran Affairs Medical Center is assigned to the Psychology Service. The PCMHI Psychologist will function within a fast-paced primary care team and can expect to consult with eight to ten patients per pay with the primary goal of assisting Patient Aligned Clinical Team (PACT) members with identification, treatment and management of mental health and behavioral medicine conditions in the enrolled population.
The PCMHI Social Worker provides education and brief interventions to patients, monitors response to and side effects from treatment, and provides education and support to patients. The PCMHI Social Worker also provides "on-time, on-target" information to Primary Care Providers (PCPs) and Behavioral Health Services (BHS) staff, and collaboratively makes appropriate care decisions.
This position is located in the Kernersville Health Care Center (HCC) in Kernersville, NC 27284. The VISN 6 TECS program is part of the VISN 6 Clinical Resource Hub (CRH) aligned under the VISN 6 Telehealth Medical Director. The TECS program is managed by the VISN 6 CRH Optometry Chief. The TECS technician is organizationally aligned to the VA Health Care System (facility) and is directly supervised by facility staff
The Registered Nurse-Primary Care Mental Health Integration Nurse are responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. The Nurse follows procedures per established policies and guidelines.
