Job Summary

This position is located in the Kayenta Service Unit's, Division of Nursing, Outpatient/Pediatric Department in Kayenta, AZ. The role provides case management and clinical coordination for patients and responsibilities include patient assessment, care coordination, service integration, follow-up, and evaluation to improve continuity of care, patient flow, and overall clinic services. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).

BASIC REQUIREMENT(S): Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education(external link), Council on Accreditation of Nurse Anesthesia Educational Programs(external link), Accreditation Commission for Midwifery Education(external link), or an accrediting body recognized by the U.S. Department of Education(external link) at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools (external link) is required for individuals who graduated from foreign nursing schools. Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below- MINIMUM QUALIFICATIONS: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Specialized experience includes: Professional nursing knowledge and skills in ambulatory care, patient assessment, case management, care coordination, chronic disease management, patient education, clinical documentation, and referral coordination. Must be able to demonstrate the ability to provide nursing care to diverse patient populations, communicate effectively with patients and healthcare teams, recognize and respond to changing patient conditions, operate clinical equipment, and collaborate with multidisciplinary providers to support continuity of care. You must meet all qualification requirements within 30 days of the closing date of the announcement.
